Jerry Horak, age 74, of Lakeland, FL passed away on January 15, 2022.
A native of [the] Czech Republic, he was the son of the late Jaraslav and Bozena Horakova.
Survivors include his daughter, Jarka Alvarez and sister Karina Horakova. He was preceded in death by his wife, Jaroslava and granddaughter Jennifer Alvarez.
Before moving to Lakeland in 2017, Jerry was a chef and owner of the Timberline Motel in Hancock, New York. He was a professional hockey and soccer player for his country, Czech Republic before moving his family to the United States. He thoroughly enjoyed bicycling and would often enter cycling tours.
Services will be held privately by the family.
